How Our Referral Process Works

Complete Intake Form
Start by submitting our online partnership inquiry or the standard NSWH Intake Form to provide basic client information.

Eligibility Review
Our team reviews the referral for medical stability, physical independence, and shared housing appropriateness.

Placement Decision
We quickly communicate our placement decision to the referring agency, usually within one business day of receiving complete records.

Coordination & Move-In
Once approved, we coordinate transportation and move-in logistics with your discharge team for a seamless transition.
Note: All referrals must meet basic criteria: medically stable, able to perform all ADLs independently, and able to self-administer medications.
